What type of practice is Paradigm by DW Medical?
Paradigm by DW Medical is a **physician-led, direct-pay concierge telemedicine practice**. Care is provided directly by Dr. Denise D. Wallis, MD, FAAFP, with an emphasis on time, continuity, and thoughtful medical decision-making.
What conditions do you treat?
Care is focused on: • Menopause and women’s health • Weight and metabolic health • Longevity and preventive medicine • Medical second opinions and care navigation • Limited virtual urgent care for select conditions Services are intentionally focused to ensure depth and continuity of care.
Do you prescribe controlled substances?
No. This practice does not prescribe controlled substances, including stimulant medications, opioid pain medications, or habit-forming sedatives.
Do you evaluate or treat ADHD?
No. ADHD evaluation, management, and treatment are not offered through this practice. Patients seeking ADHD-related care should establish with a local primary care physician or psychiatrist who can provide longitudinal, in-person follow-up.
Do you provide pain management
No. Chronic pain management and opioid prescribing are not provided.Non-opioid guidance may be offered when pain is part of a broader medical issue, but patients equiring pain management should seek care with an appropriate specialist.
Who is this practice best suited for?
This practice is ideal for patients who: • Want physician-led, relationship-based care • Value being heard and having time for discussion • Are motivated to participate in improving their health • Are navigating menopause, metabolic issues, or preventive health • Want clarity through medical second opinions Age is not a barrier. There is always something that can be done.
Is this practice appropriate for international or UAE-based patients?
**Is this practice appropriate for international or UAE-based patients?** Yes. Dr. Wallis works with U.S.-based and international patients, including those living or traveling abroad. International consultations often focus on education, interpretation of medical records, second opinions, and coordination with local clinicians, in accordance with applicable regulations.
What is a discovery call?
A discovery call is a **brief, complimentary conversation** to determine whether this practice is the right fit for your needs before scheduling a medical visit. It is not a medical consultation and no diagnoses or prescriptions are provided.
How do I get started?
Care begins with a discovery call. If there is mutual alignment, a comprehensive virtual consultation is scheduled.
Fees & Insurance
What are your visit fees?
My services are offered with transparent, upfront pricing: New Patient Comprehensive Consultation (60 minutes): $395 Includes a full review of your medical history, labs, health goals, and a personalized care plan. Follow-Up Visit (30–45 minutes): $225 Designed for ongoing management, lab review, medication adjustments, and counseling. Second Opinion / Complex Case Review: $495 Includes in-depth review of submitted medical records, a written summary, a 60-minute consultation, and a follow-up plan. Extended visit types, care packages, and preventive or longevity-focused programs are available upon request.
Do you accept insurance?
No. I do not participate in insurance networks. Insurance-based medicine often prioritizes speed over thoughtful, individualized care and tends to focus on treating illness rather than preventing it. By remaining independent, I’m able to: Spend the time needed to fully understand your concerns Create customized care plans without insurance-imposed limitations Avoid rushed, high-volume visits Offer clear, transparent pricing with no hidden costs This model allows me to focus on you as a whole person—not a billing code.
Can I still use my insurance, HSA, or FSA
Yes, in many cases. Although I do not bill insurance directly, many patients are able to: Use HSA or FSA funds to pay for visits Submit a superbill (which I can provide) to their insurance for possible reimbursement Apply out-of-network benefits, depending on their insurance plan Reimbursement is not guaranteed, so I recommend checking with your insurance provider to understand your specific benefits.
Before You Book
Please Review Prior to Scheduling
This is a direct-pay medical practice. We do not bill insurance. Payment is required at the time of booking in order to confirm your appointment. A secure credit card entry is required when scheduling online.
Appointments are reserved specifically for you. Please ensure you are available at the scheduled time.
Payment Policy
All visits are self-pay.
Full payment is collected at the time of booking through our secure scheduling platform.
We do not bill insurance. Upon request, we can provide a detailed receipt (superbill) that you may submit to your insurance carrier for possible out-of-network reimbursement. Reimbursement is not guaranteed and depends on your individual plan.
We do not participate in Medicare. If you are a Medicare beneficiary, you will be asked to sign a private contract acknowledging that neither you nor the practice will submit claims to Medicare.
Cancellation & Rescheduling Policy
Appointments may be rescheduled or canceled up to 24 hours prior to your scheduled time.
Cancellations within 24 hours and missed appointments are non-refundable.
We understand that emergencies occur. Exceptions may be considered at the discretion of the practice.
Telemedicine Policy
All visits are conducted via secure telemedicine unless otherwise specified.
You are responsible for ensuring:
A stable internet connection
A private location
A working camera and microphone
If technical issues prevent completion of the visit and cannot be resolved within a reasonable timeframe, the appointment may need to be rescheduled.
Second Opinion Policy
Second opinion consultations require:
Submission of all relevant medical records prior to the appointment
Upload of imaging, pathology reports, and lab results when applicable
A clearly defined question for review
Failure to submit records at least 48 hours before the appointment may result in rescheduling. Second opinion visits are consultative in nature and may not include prescribing, diagnostic testing, or ongoing management unless otherwise agreed upon.
Scope of Care Disclaimer

Chronic pain management
Controlled substance prescribing
ADHD stimulant prescriptions
Emergency care

GLP-1 / Weight Management Disclosure
If you are using GLP-1 medication:
Weight loss medications alter appetite and nutrient intake. Our approach emphasizes muscle preservation, metabolic health, and long-term sustainability — not appetite suppression alone.
Laboratory monitoring may be recommended based on your individual risk factors.
Mental Health Screening Notice
All new patients complete standardized screening for depression and anxiety as part of comprehensive care.
This does not replace psychiatric care. If moderate or severe symptoms are identified, further evaluation or referral may be recommended.
Licensure Notice
Medical services are provided only to patients physically located in states where the physician is licensed at the time of the visit.
By scheduling, you confirm that you will be physically located in an eligible state during your appointment.
Refund Policy
Medical services are time-based professional services. Once a visit has occurred, fees are non-refundable. Prepaid appointments canceled within the allowed cancellation window may be rescheduled.
